VA CHERP Leads Major 5-Year Implementation of Age-Friendly Health System in VISN4

VA QUERI has funded a five-year, $5 million initiative, led by VA CHERP investigators, to implement four evidence-based practices that catalyze the Age-Friendly Health System (AFHS) model in VISN 4, improving outcomes for older veterans.  VISN4 serves 300,000+ Veterans, including the fifth-largest population of older Veterans in the VHA.  

“Research shows that having unmet care needs among older adults accelerates cognitive and physical decline, leads to poorer health, and increases risks for more frequent hospitalization and nursing home use,” according to Robert E. Burke MD, MS, one of four co-principal investigators on the project.  “With 62% of the VISN4 Veteran population over the age of 65, this funding provides the opportunity to transform our care system-wide, and to create a model to share across the VA.” 

Dr. Burke's Co-Principal Investigators are Judith A. Long, MD, Rachel Werner MD, PhD, and Daniel Hall, MD, MDiv, MHSc. 

Directly responsive to numerous VA priorities, the AFHS VISN4 QUERI has strong support from central offices including Geriatrics and Extended Care, the National Surgical Office, and VISN4 leaders including the directors of all 9 VAMCs and 45 CBOCs within the VISN. The initiative will address issues across a variety of care settings, including the patient’s homes, to promote safe care, support caregivers, better meet the needs of Veterans living in the community, and reduce the number of days spent in a hospital or long-term care facility.  In addition, the project leaders will mentor the next generation of implementation scientists, develop Rapid Response Team capabilities, and broaden the inclusion of Veterans in the design and promotion of program activities. 

While the Age-Friendly Health Systems model is in place in 100 health systems across the U.S., this is the first implementation of this initiative in the VA.
